*𝐀𝐆𝐑𝐄𝐄𝐌𝐄𝐍𝐓 𝐆𝐔𝐈𝐃𝐄𝐋𝐈𝐍𝐄𝐒* PLEASE TYPE YOUR FULL NAME AND I AGREE TO CONFIRM YOUR UNDERSTANDING OF THE FOLLOWING INFORMATION: You will need to provide us with a final headcount 48 hours prior to the party. This will allow us to make any final adjustments to your party package. At that time, any additions or changes can be made to the party. After the 48 hour deadline, the package cannot be altered and the remainder of your balance will be charged the day before your party. Email events@easternathleticsnc.𝐜𝐨𝐦 to confirm your headcount! There are no exceptions. 𝐓𝐄𝐑𝐌𝐒 & 𝐂𝐎𝐍𝐃𝐈𝐓𝐈𝐎𝐍𝐒: After you submit your form, we will contact you via email or phone to confirm your reservation. At that point, we will charge a deposit of $75. This must be after you submit your completed forms. The remaining balance will be charged the day before your party. Full payment of estimated charges for the party is due the day before the party. Any charges which exceed the pre-paid party charges and deposit shall be due at the conclusion of your event. We require at least 8 days advance notice if you need to cancel or reschedule your party. If you cancel within 7 days of your event, you will be responsible for 50% of the total party price. If you cancel your party within 48 hours of your party date, you will be responsible for 100% of the total party price. Your party must be rescheduled within 1 month of the original party date. Remember that your deposit is non-refundable, but can be applied to rescheduled parties within 1 month of the original party date. *
