Toddler Dance

Leotard and tights, in any color, pink ballet slippers

Hair: Pulled back from face, preferably in a bun. If short hair, use headband.

Ballet

Boys: Black basketball or running shorts or long sweatpants (not baggy), white t-shirt or tank top (something not too loose) and black ballet slippers.

Girls: Pink tights, any color leotard, pink ballet shoes. Pointe shoes if applicable.

Hair: A secured bun using a hair net, hair pins and scrunchy. If short hair, use headband.

Jazz, Lyrical, Tap, Contemporary, Lyrical, Jumps & Turns, Stretch

Biker shorts or leggings and a t-shirt or tank top (would prefer that the top not be too loose so that the instructors can easily see the exact movements of the dancers).

Hip Hop

Sweatpants, leggings or stretchy shorts, a t-shirt or tank top, and sneakers. Hair pulled away from face.

Shoes

Tap: Tap shoes

Jazz, Lyrical and Jumps & Turns: black or tan jazz shoes or half sole’s/turners. 

Ballet: pink ballet shoes, pointe shoes with ribbon and elastic

Hip Hop: Sneakers

Tumbling: barefoot

Studio Policies
These policies are to help with the safety of students, help the studio function in an efficient manner, and, above all, provide students with an unparalleled dance experience. 

Parents, please review the policies with your child. This document outlines requirements each student will be held to during their entire duration of dancing with Captivating Dance by Nona.

Registration/Enrollment for studio classes
All parents or legal guardians of students and students 18 years or over must sign electronically the liability waiver and acknowledge understanding of this document before the student is allowed to participate in class. Every fall, all families will be asked to electronically sign a new waiver.

There is a non-refundable, $50.00 yearly registration fee due once a year.  Credit card on file will be charged in addition to your first month tuition.

We do not require prerequisite classes to take dance.  If you are not sure which level is appropriate for your child, please contact us.

Please notify us as soon as possible with any changed information to your enrollment (contact information [phone, email], emergency information, medical status, etc.)  Our main form of communication is our monthly newsletter that will be emailed monthly.  Contact us if you would like to add another email address for correspondence.

Tuition
If you would like to try an OPEN class before enrolling, you may do so at $25 per class. You will still be required to sign enrollment and liability electronic waiver before you can participate.  Drop-in classes must be paid before participating in the class.

Tuition is non-refundable.  Monthly tuition is due on the 1st day of every month. It is considered late if not paid by the 5th day of the month. There will be a $20 late fee applied to your account on the 11th day of the month. No exceptions. Tuition is non-refundable.

You are responsible for all monthly tuition for each month from August – May, unless you have given us a 30-day written email notice to withdraw from class(s). 

Full monthly tuition is due regardless of any missed classes. Monthly tuition rates remain the same whether it is a long month or short month. 

Our studio classes culminate in an end of the year dance recital in late May. There are no OPEN studio classes following the May recital. We do not accept new students after February, as students will be taught the dance routines for the recital in the months of Feb-May. 

Tuition will be charged to the credit card on file. There is a $30 fee for returned checks or declined credit card transactions due to the bank fees incurred by Captivating Dance by Nona and accounting staff. There will not be a statement or bill given to you monthly for tuition unless your account is past due or we have received a non-sufficient payment. Student accounts need to be current in order for student to participate. 

CDBN has the right to charge any credit cards signed up for auto payment or card left for delinquent accounts. All payments are final and non-refundable.

Recital Fee

 A $85 recital fee will be automatically charged to the card on file.  This charge will be in 2 installments - $85 in October for Winter Showcase and $85 in February for Spring Recital.  All enrollments after November/February will be charged $85 upon enrollment.  This fee is non- refundable.

Attendance/Drop Off & Pick-Up Policy
Attendance is taken at each class. Good attendance is imperative, as absences and tardiness can hold back an entire class. The studio can’t jeopardize its responsibilities to the rest of the class for one student.

If you miss a class you will not receive a credit or refund, however, as a courtesy, you will be able to makeup the missed class within 2 weeks of the missed class(s). You must contact the studio to arrange a make-up class.

If a student must miss a class or will be late for any reason, the student (or parent) must communicate this to the appropriate instructor before the scheduled class begins.

If we need to cancel class for any reason, we will reschedule the cancelled class within 30 days of the cancelled class.

If within the first 15 minutes of any given class, no students (non-CDBN competition dancers) arrive, the instructor has the right to cancel the class for OPEN students.

Please be prompt in dropping off and picking up your student. Students must be no later than 5 minutes to class or they may not be granted permission to attend that class. Students must be picked up no later that 10 minutes after class. Please notify the studio if you cannot get your student to or from class on time. This applies to rehearsals and performances.

For your child’s safety, ensure your student is with an instructor and in a lesson before leaving.

CDBN General Release
As the Parent or Legal Representative, acknowledge that my daughter, son, and/or child for whom I have legal custody has voluntarily applied to engage in dance and dance-related programs and activities with Captivating Dance by Nona.  I understand I should be aware of my child’s physical limitations and agree to inform the studio of such limitations.    
                                                
I understand that participation in dance class, practice, workshop, camp, intensive, rehearsal, performance, competition or other dance activity is potentially risky.  I knowingly and voluntarily agree to assume all reasonable risks and responsibilities for any such injury or accident, which might occur to my child or me before, during, or after any Captivating Dance by Nona dance or dance-related activity that my child or myself has chosen to participate in except for those resulting from negligence or willful misconduct on the part of Captivating Dance by Nona or any of its agents. I understand that Captivating Dance by Nona is not responsible for any injury or accident that occurs under such circumstances unless it arises due to negligence or willful misconduct on the part of Captivating Dance by Nona or any of its agents.

I am aware that my child may on occasion be driven by an agent of Captivating Dance by Nona either between studio or to or from a competition, convention or other dance-related activity as a convenience to parents and I understand that Captivating Dance by Nona is not responsible for any injury or accident that occurs under such circumstances unless it arises due to negligence or willful misconduct on the part of Captivating Dance by Nona or any of its agents.

I knowingly and voluntarily exempt, release, indemnify, and hold harmless Captivating Dance by Nona, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, independent contractors, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes, activities or events conducted by Captivating Dance by Nona, except for those resulting from negligence or willful misconduct on the part of Captivating Dance by Nona.  I further hereby knowingly and voluntarily agree to waive my rights and that of my heirs and personal representative(s) to hold Captivating Dance by Nona, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, independent contractors, and/or students liable for such damage, loss, injury, or death.                                                 
Media Release & Authorization 
I hereby give Captivating Dance by Nona permission to use still or video images of my child dancing with Captivating Dance by Nona in any reasonable form of advertisement for the studio. 

Medical Authorization 
I hereby give Captivating Dance by Nona permission to seek emergency treatment for my child and I authorize Captivating Dance by Nona representatives to give permission for the performance of emergency medical and dental examination(s) and necessary treatments (including tests, x-rays, medication or emergency procedures) as deemed necessary by the physician/dentist in attendance. If any emergency arises, Captivating Dance by Nona representative will attempt to reach me and/or my child’s emergency contacts and be guided by given instructions. If I cannot be reached, I authorize the attending physician/dentist to act as reasonable medical judgment dictates. I agree to accept financial responsibility for the costs/debts related to any and all emergency treatment deemed necessary by the physician/dentist in attendance.  In the event that my child has a special medical need such as a life-threatening allergy or a disease, which requires monitoring or medication, I will make appropriate arrangements in advance and provide all necessary information to Captivating Dance By Nona.

Dancing is a strenuous activity from which injuries could arise. Captivating Dance by Nona, staff, and employees are NOT LIABLE for personal injuries, loss of, or damage to personal property. Each student may decline to participate in any activity. Please inform the instructor of any physical limitations you may have. If you are in doubt as to your physical abilities, please consult your physician before participating. Captivating Dance by Nona can not dispense of aspirin or other medication. 

In the event of an emergency, Captivating Dance by Nona is authorized to secure emergency medical treatment which you will be responsible for. CDBN retains the right to use photos and or video in any way they see fit without compensation. CDBN has the right to charge any credit cards signed up for auto payment or card left for delinquent accounts. All payments are final and non refundable. The views of Captivating Dance by Nona may not be the views of all affiliated sponsors. I have read and understand all studio policies. If your child is under the age of 18 you will explain this release to your child.

There are no refunds for missed classes due to studio holiday and/or breaks and/or Inclement weather closure.  All make up classes must be done within the same month of the absence.
